Admission Procedures

Your relative will be requested to:

1. Present your doctor’s order for admission. For walk-in patients, the Hospitalist on Duty will make an admitting order after taking your history and physical examination.
2.  Fill up the admission form. The information requested is designed to make your records as complete and accurate as possible. This is a vital procedure and we appreciate your cooperation.
3. Select the type of room accommodation that you want. Your room accommodation may be dependent on your case which will be determined by the Hospitalist on Duty.
4. Sign the consent form for the hospital care and acceptance of financial responsibility and other forms needed.
5. Wear your ID band.
